One of the more common questions I get asked, is what’s the difference between a TURP and a Greenlight Laser Prostatectomy
If you're reading this, you're likely facing bothersome urinary symptoms due to an enlarged prostate — a common condition that many men experience as they age. Whether it's frequent urination, a weak flow, or the urge to rush to the bathroom, these symptoms can significantly impact your quality of life.
Two highly effective surgical options exist to address these issues:
TURP (Transurethral Resection of the Prostate) and the Greenlight Laser Prostatectomy.
This newsletter aims to help you better understand the differences between these two treatments, so you can choose the path that best suits your needs.
Why Is Treatment Needed?
As men age, the prostate — which sits beneath the bladder and surrounds the urethra (urine tube) — often grows larger. This condition is known as Benign Prostatic Hyperplasia (BPH).
As the prostate enlarges, it can obstruct the flow of urine, causing symptoms like:
Frequent urination, especially at night
Difficulty starting urination
Weak urine stream
Incomplete bladder emptying

When lifestyle changes and medications no longer provide relief, surgical options may be considered.
What Is TURP?
TURP is a time-tested surgical method used for decades. It involves:
A telescopic camera inserted through the penis
Electric current is used to shave away prostate tissue
Performed under general or spinal anaesthesia
Typically involves a hospital stay of 1–2 nights
A catheter is used post-op to drain the bladder for a few days
Pros:
✅ Very effective at improving urinary flow
✅ Long-established method with proven results
Cons:
⚠️ Higher risk of bleeding (may require blood transfusion in rare cases)
⚠️ Greater chance of sexual side effects, including:
Retrograde ejaculation is a common
Erectile dysfunction (affects 7–20% of men)
What Is Greenlight Laser Prostatectomy?
Greenlight Laser Prostatectomy is a more modern, minimally invasive technique. It uses a 532-nanometer laser that targets blood-rich prostate tissue while minimising damage to surrounding areas.
Done through a telescopic camera via the penis
Uses laser energy to vaporise excess tissue
Performed under general anaesthesia
Usually a 1-night hospital stay
Most men go home without a catheter the next day
Pros:
✅ Lower risk of bleeding (blood transfusions are rarely needed)
✅ Less likely to affect erectile function (less than 1% risk)
✅ Shorter hospital stay and quicker recovery
Cons:
⚠️ Retrograde ejaculation is a known and expected outcome of this procedure
⚠️ Slight chance (~10% over 10 years) of needing retreatment
How Do They Compare?
Feature | TURP | Greenlight Laser |
---|---|---|
Hospital Stay | 1–2 nights | Usually 1 night |
Catheterization | Longer | Shorter |
Risk of Bleeding | Higher | Very Low |
Erectile Dysfunction Risk | 7–20% | <1% |
Retrograde Ejaculation | Common | Common |
Durability of Results | 10+ years | 10+ years |
Time in Use | 40+ years | 20+ years |
Final Thoughts: One Size Doesn’t Fit All
Both TURP and Greenlight Laser procedures are excellent options for relieving urinary symptoms caused by an enlarged prostate. The right choice for you will depend on:
Your medical history
Your personal preferences (e.g., concern about sexual side effects)
Your recovery expectations
Any bleeding risks or anaesthesia considerations
